Amends 63 O.S. 2011, Section 1-132 to establish the Oklahoma Health Information Exchange Trust (OHIET) as a public trust with the state as beneficiary, contingent upon finalization and adoption of a declaration of trust.

Designates OHIET as Oklahoma's "Qualified State-Designated Entity" to receive federal grants under 42 U.S.C. Section 300jj-33 for expanding electronic health information exchange among organizations using nationally recognized standards.

Establishes a seven-member board of trustees appointed by the Governor (3 members), President Pro Tempore of the Senate (2 members), and Speaker of the House (2 members), with staggered terms of 1-5 years and subsequent five-year terms.

Requires OHIET trustees to establish an advisory board with representatives from healthcare providers, health plans, patients, technology vendors, employers, public health agencies, educational institutions, researchers, and other health information technology users.

Terminates OHIET provisions on January 1, 2016, with the Oklahoma Health Care Authority assuming responsibility for managing federal grants awarded prior to the act's effective date; act becomes effective November 1, 2015.
