Requires the Adult Protective Services Program Office of the Department of Human Services to immediately file a guardianship petition when it determines an incapacitated person requires transfer to a nursing home or other living arrangement.

Prohibits employees of the Adult Protective Services Program Office from being eligible for appointment as guardian to any person who is the subject of a petition filed by the Office.

Requires investigations and background checks for petitioners and adult household members, including criminal history checks through local records and the Oklahoma State Bureau of Investigation (OSBI) criminal history repository.

Mandates petitioners disclose the case name and status of any civil or criminal matters in state or federal court involving the petitioner or adult household members, including bankruptcy adjudications.

Becomes effective November 1, 2015.
