Amends Oklahoma law to require advance directives include a specific acknowledgment that "the withholding of water can result in a painful death."

Advance directives must be signed by the declarant and witnessed by two individuals age 18 or older who are not legatees, devisees, or heirs at law.

Any decisions to withhold or withdraw artificially administered nutrition and/or hydration must be specifically authorized in the declarant's own words or in a separately initialed, signed, or marked section dealing only with nutrition and/or hydration.

Includes comprehensive form for advance directives covering living will instructions, health care proxy appointment, anatomical gifts, and general provisions regarding revocation and physician decision-making standards.

Becomes effective November 1, 2015.
