Mandates confirmation by both an attending physician and consulting physician of the terminal diagnosis, patient capability, and voluntary decision-making; requires psychiatric/psychological counseling referral if impaired judgment is suspected

Provides immunity from civil, criminal, and professional liability for health care providers participating in good faith; allows providers and facilities to opt out of participation

Establishes felony penalties for forging requests, concealing rescissions, or coercing patients; prohibits insurance discrimination based on participation; requires State Department of Health to collect compliance data and issue annual statistical reports
