School districts must include employee information worksheets for teachers and administrators containing three categories: employee information (name, degrees, years of teaching credit, salary schedule placement), salary information (state minimum pay, district supplements, extra-duty pay, fringe benefits), and benefits information (flexible benefit allowance, retirement contributions, health insurance, other district-paid benefits).

School districts must annually provide each teacher and administrator a copy of their employee information worksheet prior to the first payroll in September, beginning with the 2016-2017 school year.

School districts must designate one or more persons to review worksheets with teachers or administrators upon request and answer any questions about the information.

The State Department of Education must require amounts paid in each salary and benefits category to be reported annually in its personnel report, disaggregated by administrative personnel and certified teaching personnel.
