Insurers must be willing to enter into provider service agreements with qualified health care providers who meet insurer requirements and practice within the insurer's service area

Covered health care providers include podiatric physicians, chiropractic physicians, dentists, pharmacies, allopathic physicians, physical therapists, optometrists, and osteopathic physicians

Insurers are not required to contract with providers who fail to meet insurer requirements or practice outside the insurer's geographic service area
