Reduces the employer size threshold for coverage from 50 employees to 20 or more full-time or full-time-equivalent employees, expanding the number of employers subject to reporting requirements.

Requires employer carriers to provide written reports on request containing 12-month data on charges submitted, premium payments, carrier payments to providers, and individual claims exceeding $10,000 with diagnostic information.

Establishes a two-tier timeline for information delivery: 60 days before plan anniversary/renewal or 30 days before rate changes, with a 60-day deadline if requests are received with less notice.

Allows carriers to waive penalties if a delegated medical group, IPA, or MSO fails to provide information timely, protecting carriers from liability for third-party delays.

Imposes civil penalties of $100 per day for delinquent information delivery, enforceable by the Insurance Commissioner, with penalties placed in the State Insurance Commissioner Revolving Fund.

Becomes effective November 1, 2017.
