Requires any physician performing an abortion on a minor under 14 years of age to preserve fetal tissue in accordance with Oklahoma State Bureau of Investigation rules and submit the tissue to that agency.

Mandates the State Bureau of Investigation adopt rules specifying the amount and type of tissue to preserve, DNA testing procedures, chain of custody documentation, tissue disposal procedures, and a uniform reporting form including physician and parent/guardian contact information.

Requires the reporting form to include the name and complete address of the parent or legal guardian of the minor on whom the abortion was performed.

Establishes that physician failure to comply with preservation and submission requirements constitutes unprofessional conduct under Section 509 of Title 59 and is a felony offense.

Takes effect November 1, 2017.
