• Establishes coverage requirements: females ages 35-39 entitled to low-dose mammography screening once every five years; females age 40 and older entitled to annual screening.

• Adds "supplemental examination" coverage for medically necessary screening based on personal or family medical history, or risk factors including dense breast tissue.

• Becomes effective November 1, 2025, with health savings account compliance provisions for plans with high deductibles under federal tax code.
