Prior authorization for non-inpatient care treating chronic conditions remains valid for at least 6 months from approval date, unless clinical criteria changes with notice provided.

Prior authorization for inpatient acute care treating chronic conditions remains valid for at least 14 calendar days from approval date.

For inpatient care extensions beyond the initially approved length of stay, utilization review entities must evaluate continuation requests using the same criteria as the initial request and consider relevant medical literature provided by the health care provider.

If a utilization review entity fails to respond to a timely continuation request (submitted at least 24 hours before approval termination), the health benefit plan must continue compensating the provider at the contracted rate until a determination is issued.

The bill becomes effective November 1, 2025.
