Amends the definition of "covered services" in 36 O.S. 2021, Section 7301 to clarify that services are reimbursed under subscriber agreements notwithstanding contractual limitations such as deductibles, waiting periods, or frequency limitations.

Requires health benefit plans and dental plans to establish and maintain appeal procedures for claims denied based on lack of medical necessity.

Mandates that any denial of benefits for medical necessity reasons must be determined by a dentist holding a nonrestricted license in the United States.

Requires written communications denying claims to include the reviewing dentist's identifier, license number, state of issuance, and contact telephone number, with contact limited to business hours.

Effective date: November 1, 2025.
