• Nursing facilities, residential care facilities, assisted living facilities, adult day care centers, and continuum of care facilities that advertise dementia or memory care must disclose the specific types of care and treatment provided using a standardized form developed by the State Department of Health.

• Referral agencies must provide representatives of persons with Alzheimer's dementia or other forms of dementia with electronic or physical access to disclosure forms for each facility when considering placement, and the Department of Health must maintain a public searchable database of forms.

• Facilities must disclose nine required areas of information including overall philosophy and mission, placement criteria, staff-to-resident ratios, staff training, physical environment features, resident activities, family involvement, fees, and accreditations or certifications.

• Facilities offering specialized memory or dementia care must post the disclosure form on their website and display a poster in conspicuous locations stating that unserved persons or representatives may file complaints with the Consumer Protection Unit of the Office of the Attorney General.

• The Department of Health shall establish a website listing facilities that filed disclosure forms with links to submitted forms and a table containing staffing ratios, training, programming, and accreditations; violations are subject to notice and enforcement provisions established for the facility's license; effective November 1, 2025.
