Creates the "Vaccine Transparency and Informed Consent Act" requiring health care providers to give patients written documentation before any vaccine administration, including complete ingredient lists, clinical trial summaries, known and potential side effects from trials and VAERS data, ethical considerations about fetal tissue or animal-derived components, exemption information, and National Vaccine Injury Compensation Program details

Mandates written informed consent from patients acknowledging receipt of materials, voluntary decision to accept/decline/defer vaccination, and confirmation they had opportunity to ask questions

Imposes civil penalties of up to $1,000 per occurrence for failing to provide required disclosure documentation and up to $5,000 per occurrence for administering vaccines without written informed consent

Authorizes repeat violators to face disciplinary action from the Oklahoma State Board of Medical Licensure and Supervision, including potential license suspension or revocation

Requires signed consent forms to be retained in patient medical records for minimum of 7 years, with effective date of November 1, 2025
