Health benefit plans offering contraceptive drug coverage must allow enrollees to obtain a 3-month supply on first fill, then a 6-month supply on subsequent refills of the same drug

Contraceptive drugs covered include all FDA-approved pregnancy prevention medications administered orally, transdermally, or intravaginally, but exclude medications that could terminate an existing pregnancy

Enrollees may request smaller supplies, and providers may prescribe smaller amounts when supported by clinical utility and medical appropriateness

Coverage is not required for contraceptive drugs not intended for regular or routine use
