Referral agencies must provide electronic or physical access to dementia care disclosure forms for representatives of persons with Alzheimer's or other dementia diagnoses considering facility placement

Facilities offering specialized memory or dementia care must post their disclosure form on their website and display a poster informing residents they can file complaints with the Attorney General's Consumer Protection Unit if not given the disclosure form

Enforcement penalties now apply to both facilities and any individuals employed or contracted by those facilities who violate the disclosure requirements

State Department of Health must maintain a public searchable database of disclosure forms and establish a website listing facilities that have filed disclosures, including staffing ratios, training, programming, and certifications
