Creates the Oklahoma Health Care Transparency Initiative beginning July 1, 2027, establishing an all-payer claims database to collect medical, dental, and pharmaceutical insurance claims data from health insurers and benefit plans with at least 2,000 covered individuals.

Requires submitting entities to report claims data, unique identifiers, and demographic information to the state-designated health information exchange entity, with implementation beginning by July 1, 2026.

Mandates the State Department of Health to submit public health data including hospital discharge records, birth and death records, and disease registry data for integration into the transparency database.

Protects submitted data as confidential and exempt from Oklahoma Open Records Act disclosure, while allowing use by insurers, employers, researchers, state agencies, and health care providers for assessing health care utilization, costs, and performance.

Authorizes the Insurance Department to fine non-compliant submitting entities up to $1,000 per day per violation, with proceeds funding the initiative's operations through the Oklahoma Health Care Authority.
