Oklahoma Health Care Authority must apply for federal waivers or state plan amendments by January 1, 2027, to establish a supplemental reimbursement rate for providers in the patient-centered medical home program who use team-based, evidence-based pediatric or family practice transformation models

Supplemental reimbursement covers fee-for-service wellness visits for children from birth until their fourth birthday and supports hiring interdisciplinary staff to implement screening requirements at 9, 18, and 30 months per American Academy of Pediatrics Bright Futures guidelines

Participating providers must work with data-tracking entities to demonstrate adherence to evidence-based pediatric practice transformation models focused on young children and families

OHCA must maintain annually updated verification letters confirming each enrolled provider actively participates in a qualifying team-based care model
