Prohibits Oklahoma state-issued health insurance coverage (including Medicaid, SoonerCare, and state group health plans) from covering or reimbursing gender transition procedures for both minors and adults, effective November 1, 2026

Expands existing law that already bans health care providers from performing gender transition procedures on minors (persons under 18) by adding the state insurance coverage prohibition

Defines gender transition procedures as surgeries altering sex-typical physical characteristics, puberty-blocking drugs, and cross-sex hormones intended to affirm a gender identity inconsistent with biological sex

Excludes from the definition: mental health counseling, treatment for precocious/delayed puberty, services for individuals with disorders of sex development (DSD), treatment of complications from prior procedures, and emergency care

Maintains existing penalties for providers who violate the minors ban: felony charges (prosecutable until the child turns 45), professional discipline, and civil liability allowing compensatory damages, punitive damages, and attorney fees
