Reimbursement for licensed optometric physicians must be at least the 60th percentile of usual and customary charges for the same services in the same geographic region, as determined by an independent nonprofit using private insurance claims data

Increases in reimbursement for covered services cannot be offset by decreases in reimbursement for ophthalmic materials (frames, lenses, contacts), unless changes apply uniformly to all providers including those owned by the vision plan

Vision plans cannot reduce reimbursements to providers who use nonaffiliated labs or frame vendors, provided those vendors meet credentialing standards

Vision plans must disclose average reimbursements to both affiliated and independent providers for services and materials
