Insurers are required to pay additional base unit values for patients with severe health conditions: 1 base unit for ASA III (severe systemic disease), 2 base units for ASA IV (life-threatening systemic disease), and 3 base units for ASA V (moribund patients)

Applies to insurance companies, HMOs, group hospital service corporations, nonprofit health corporations, and small employer health benefit plans

Physical status classifications must be determined by the patient's treating physician or health care provider
