Eliminates the Alzheimer-Dementia Disclosure Act Advisory Council, a 9-member body that previously advised the State Commissioner of Health on disclosure forms and rules for dementia care facilities

Transfers sole authority to the State Department of Health for developing standardized disclosure forms and promulgating rules under the Alzheimer's Dementia and Other Forms of Dementia Special Care Disclosure Act

Updates statutory terminology, changing "assisted living facility" to "assisted living center" and "memory care" references to "special care" throughout the Act

Maintains all existing disclosure requirements for facilities advertising dementia or memory care services, including submission to the Department, patient representatives, and the State Long-Term Care Ombudsman
