Adds a new definition of "acquisition cost" to include purchase price, procurement fees, and shipping/handling charges, excluding post-purchase rebates or discounts not applied at time of sale

Prohibits pharmacy benefits managers (PBMs) from refusing additional documentation from providers after appeal submission and requires PBMs to accept electronic batch format appeals without requiring manual login systems

Requires approved reimbursement appeals to apply retroactively to all prescriptions of the appealed drug under the same BIN and PCN, with full payment within 30 calendar days or automatic remittance without requiring provider refunds

Establishes minimum reimbursement floor for providers at the national average drug acquisition cost plus a professional dispensing fee at least equal to the Medicaid fee-for-service rate; if NADAC unavailable, wholesale acquisition cost applies

Requires all reimbursement adjustments to include complete claim-level detail identifying original and revised payment values; adjustments without contemporaneous detail are deemed incomplete. Effective November 1, 2026
