Creates the Uniform Health Care Decisions Act of 2025 in Oklahoma, establishing standards for health care decision-making, advance directives, and powers of attorney for health care

Presumes all individuals have capacity to make health care decisions unless a court finds otherwise or the presumption is rebutted by a contemporaneous examination by a physician, psychologist, APRN, physician assistant, or social worker

Authorizes individuals to create health care instructions expressing preferences for future care, including choice of providers, decision-making processes, guardian nominations, and default surrogate designations

Allows creation of health care power of attorney to appoint an agent for health care decisions, requiring a written record signed by the individual and an adult witness; disqualifies nursing home employees from serving as agents unless they are family members

Permits remote witnessing of power of attorney documents through real-time audio-visual electronic means or audio-only connection with identity authentication; effective date November 1, 2025
