Oregon Health Authority must expand Oregon State Hospital capacity to meet the projected need of 486 new inpatient psychiatric beds identified in the June 2024 Behavioral Health Residential+ Facility Study, with annual reporting to the Legislature beginning December 31, 2025

Courts may order defendants found unfit to proceed to participate in in-custody jail-based restoration treatment programs as an alternative to state hospital commitment, with required evaluations within 60 days and progress reports every 180 days

Defendants ordered to community restoration services must be evaluated within 60 days, with status reports to the court within 48 hours if noncompliance occurs (missed appointments, positive drug tests, new law enforcement contact, or refusal of medications)

Oregon Public Guardian and Conservator must establish a guardianship program for defendants whose criminal cases are suspended or dismissed due to lacking fitness to proceed, with access to defendant records from state hospitals and courts

Expands criteria for commitment of "extremely dangerous persons with mental illness" to include developmental disabilities, intellectual disabilities, traumatic brain injury, and brain damage as qualifying mental disorders, and adds a catch-all category for "any other act that constitutes an extreme danger"
