Requires dental laboratories operating in Oregon or serving Oregon-licensed dentists to register with the Health Licensing Office beginning July 1, 2026, with annual renewal and assigned registration numbers required on all invoices and correspondence.

Mandates dental laboratories provide dentists with material content disclosures including the laboratory's registration information, country of origin where work was performed, and FDA registration numbers or manufacturer/brand names of all patient contact materials for inclusion in patient records.

Establishes eligibility requirements for registration: laboratories must employ a certified dental laboratory technician (certified by the National Board for Certification in Dental Laboratory Technology) working at least 30 hours per week, or be supervised by a dentist working at least 30 hours per week.

Creates exemptions for dental laboratories within dental offices that only serve the supervising dentist's own patients and for educational program laboratories, though in-office labs must still file annual reports with the Health Licensing Office.

Authorizes the Health Licensing Office to impose disciplinary actions and civil penalties up to $5,000 per violation, classifies operating without registration as a Class B violation, and allows the office to seek injunctions against violators.
