Prohibits pharmacy benefit managers (PBMs) from engaging in spread pricing, imposing post-sale fees on pharmacies, or retroactively denying/reducing payment on adjudicated claims except for fraud, duplicate payment, or clerical errors

Requires PBMs to pay pharmacies a professional dispensing fee no less than the Oregon Health Authority's established rate and reimburse drug ingredient costs at the lesser of the pharmacy's usual charge or the National Average Drug Acquisition Cost

Mandates PBMs offer identical contract terms to network and out-of-network pharmacies, and prohibits contracts that require pharmacies to participate in other contracts, meet unreasonable accreditation burdens, or provide services below drug acquisition cost

Allows policyholders to select any licensed pharmacy or pharmacist for prescription drug dispensing at time of issuance, amendment, or renewal of health insurance policies

Expands pharmacy appeal rights to include challenges to ingredient costs and dispensing fees, with appeals required to be completed within 30 calendar days and responses provided within seven business days
