Prohibits abortion after 15 weeks gestational age except in cases of medical emergency, rape, or incest, and requires health care providers to determine gestational age before any abortion procedure

Mandates that abortions at 15+ weeks be performed only in licensed hospitals or hospital-owned ambulatory surgical centers, prohibits telemedicine for such procedures, and requires a second provider present to care for any child born alive

Requires health care providers to report all abortions to the Oregon Health Authority, including gestational age, method used, and whether the procedure provided the best opportunity for fetal survival; Oregon Health Authority must publish annual abortion statistics

Allows the pregnant person or the person responsible for fertilization to sue providers for actual and punitive damages for violations; also permits injunctive relief actions by prosecutors, the Attorney General, or affected parties

Adds violations to grounds for professional disciplinary action against physicians, physician assistants, and nurse practitioners; takes effect 91 days after legislative adjournment
