Establishes the Center for Decision-Making Supports within the office of the Long Term Care Ombudsman

Defines "supported decision-making" as an individualized process to help adults with disabilities make life decisions while preserving self-determination, where supporters assist but do not have authority to make decisions on behalf of the individual

Requires the center to develop accessible information systems about guardianship, supported decision-making, and alternatives to guardianship for individuals needing support

Directs the center to serve as a one-stop educational resource covering youth transition to adulthood, guardianships, health care directives, powers of attorney, and guardianship alternatives for adults with disabilities and their families

Authorizes the center to provide referrals to advocacy organizations and legal assistance, enhance cross-agency collaboration on guardianship issues, and make all information available online
