Prohibits abortion after 15 weeks gestational age except in cases of medical emergency, rape, or incest, and requires health care providers to determine gestational age before performing any abortion

Abortions at 15+ weeks must be performed in licensed hospitals or hospital-owned ambulatory surgical centers, cannot use telemedicine, require a second provider present to care for any child born alive, and must use methods providing the greatest opportunity for fetal survival

Creates civil liability allowing the pregnant person or the person responsible for fertilization to sue providers for actual and punitive damages for violations; also allows injunctive relief actions by prosecutors and the Attorney General

Requires providers to report each abortion to the Oregon Health Authority with details including gestational age, method used, and basis for any exception; intentional or reckless failure to report constitutes unprofessional conduct subject to license discipline

Takes effect on the 91st day following adjournment of the 2025 legislative session and amends ORS 435.210 to make the fundamental right to terminate pregnancy subject to the new limitations
