Requires all individual and group health insurance policies to cover orthotic and prosthetic devices, including devices medically necessary for physical activities like running, biking, swimming, and strength training

Mandates coverage for replacement of devices when the insured's physiological condition changes, the device is irreparably damaged, or repair costs exceed 60% of replacement cost

Prohibits insurers from denying prosthetic or orthotic coverage for individuals with limb loss if the same physical activity restoration would be covered through medical or surgical intervention for someone without limb loss

Requires managed care organizations to provide access to at least two distinct Oregon prosthetic and orthotic providers in their network

Applies to policies issued, renewed, or extended on or after January 1, 2026, with an exemption for Public Employees' Benefit Board and Oregon Educators Benefit Board plans unless they elect to provide coverage
