Establishes the Medicaid Care Transition Program within the Department of Human Services to address delays in transferring Medicaid patients from hospital emergency departments and inpatient units to appropriate postacute treatment settings

Requires responsible entities (Medicaid managed care plans) to establish care transition units within 180 days and seek out-of-network placements if in-network options are unavailable within 24 hours of a request for assistance

Mandates responsible entities notify the Department of Human Services if placement cannot be secured after exhausting network options or after 48 hours from the assistance request

Directs the department to intervene on unresolved cases by convening discussions with clinical leaders, other state agencies, and the Insurance Department to address clinical, administrative, or payment barriers

Requires annual reporting to legislative committees on escalation instances, department interventions, patient diagnoses, hospital stay lengths, and corrective actions taken
