Establishes the Reproductive Health Services Address Confidentiality Program within the Department of Health, providing confidential substitute addresses (post office boxes) to reproductive health service providers and patients who fear violence from an assailant or another individual

Program certification lasts three years and requires applicants to file an affidavit describing threatened or actual violence, designate the department as agent for service of process, and provide actual address and pending legal proceedings information

State and local government entities must accept the substitute address in lieu of actual addresses, except when granted a waiver by the department or when the participant is a released offender on parole/probation or a registered sex offender

Department records are exempt from Right-to-Know Law requests, and actual addresses may only be disclosed through a formal waiver process, court order, or emergency situations requiring 24-hour access

Criminal penalties include second-degree misdemeanor for fraudulently accessing a participant's actual address (upgraded to third-degree felony for repeat offenses or those with prior violent crime convictions against the participant), and summary offense for unauthorized disclosure of addresses obtained through the waiver process
