Authorizes the Insurance Department, in consultation with the Department of Health, to add new immunizations to coverage requirements based on Advisory Committee on Immunization Practices recommendations or medical evidence showing public health benefits

Permits removal of immunizations from coverage requirements if the Department of Health determines they are no longer supported by medical evidence or effective at reducing infectious disease spread

Allows insurers to impose cost-sharing for out-of-network immunizations, except when no in-network provider is available to administer the vaccine

Establishes enforcement penalties including fines up to $5,000 per violation ($10,000 for willful violations), with an annual cap of $500,000 per insurer, plus potential license suspension or revocation
