Establishes the Office of Health Equity within the Pennsylvania Department of Health to reduce health disparities and address social determinants of health, staffed with a director, epidemiologist, biostatistician, and support personnel

Creates the Health Equity Advisory Committee to advise the Governor and Secretary of Health on programs to address health disparities, with membership including representatives from academia, healthcare providers, community organizations, and disparate populations

Requires the Office to collect and analyze data from public entities, coordinate research on health disparities, develop evidence-based policies, and produce annual reports on health disparity status and strategies

Mandates all public entities (Commonwealth agencies, municipalities, school entities, and their contractors) to cooperate with the Office and provide necessary data while protecting personal identifying information

Establishes third-degree misdemeanor penalties for unauthorized release or possession of protected data, with the act taking effect 60 days after enactment
