The sample motion form is not intended to be the exclusive form accepted by courts of common pleas, allowing flexibility in how individuals file

"Law enforcement agency" is defined to include Pennsylvania State Police and local police departments of cities, boroughs, incorporated towns, townships, and campus police

The act takes effect 60 days after enactment
