Establishes fatality review teams in each Pennsylvania agency to review deaths of older adults that occur during active investigations of abuse or neglect allegations

Teams must include 9 designated members: agency director, agency employee, advisory board member, physician specializing in elder care, victim services representative, local law enforcement, behavioral health representative, crisis intervention representative, and an elder law attorney

Fatality review teams must submit written reports to the department within 90 days of commencing an examination, including deficiencies identified, recommendations for improving protective services, and proposed solutions for system inadequacies

Department must compile annual summary reports by November 1 each year for the Governor and General Assembly, with redacted versions made publicly available online unless a district attorney certifies release would compromise a criminal investigation

Takes effect 120 days after enactment and grants teams broad authority to access health records, court documents, and agency files without requiring authorization from the deceased's estate
