Health insurance policies in Pennsylvania would be required to cover infertility care and waive all cost-sharing requirements (deductibles, copays, coinsurance) for these services

Covered treatments include IVF, intrauterine insemination, egg/sperm/embryo cryopreservation, surrogacy costs, fertility medications, and unlimited embryo transfers following American Society for Reproductive Medicine guidelines

Coverage must extend to spouses and dependents without discrimination based on age, gender identity, sexual orientation, marital status, or other protected characteristics

Insurers are prohibited from imposing preexisting condition exclusions, using prior infertility diagnosis to restrict coverage, or setting arbitrary limits on the number of treatment attempts or costs

Applies to individual and group health plans, CHIP, Medicaid managed care, and HMOs but excludes accident-only, dental, vision, long-term care, and similar limited policies; takes effect 60 days after enactment
