Health insurance policies providing pregnancy-related benefits must cover diagnosis and treatment of infertility, including in vitro fertilization, embryo transfer, artificial insemination, and related procedures

Coverage may be limited to 3 assisted reproductive technology procedures per covered individual, and insurers may impose waiting periods consistent with American Society for Reproductive Medicine or American College of Obstetricians and Gynecologists guidelines

Insurers may exclude coverage for reversal of elective sterilization, women beyond childbearing years, and may restrict assisted reproductive techniques to policyholders and dependent spouses only

Religious institutions and organizations may exclude these procedures from policies if coverage violates their religious and moral teachings

Infertility is defined as inability to conceive after one year of unprotected intercourse or inability to sustain a successful pregnancy; the act takes effect in 60 days
