Prohibits health insurers from establishing annual or lifetime dollar limits on "core benefits" for enrollees, whether services are provided by in-network or out-of-network providers

Defines core benefits as those for which no annual or lifetime limits were permitted in individual or small group policies first offered in Pennsylvania in 2018, plus any benefits specified by future department regulations

Applies to most health insurance policies but excludes accident-only, dental-only, vision-only, Medicare supplement, workers' compensation, and similar limited-benefit policies

Authorizes fines of up to $5,000 per violation (or $10,000 for willful violations), with aggregate annual caps of $500,000 for insurers and $100,000 for other persons, plus potential license suspension or revocation

Takes effect immediately for policies requiring rate/form filings with the Insurance Department, or 180 days after enactment for policies not requiring such filings
