Establishes the Maternal Health Monitoring Pilot Program within the Department of Health for fiscal years 2026-2027 and 2027-2028, providing remote patient monitoring for maternal hypertension and diabetes to pregnant Medicaid recipients

Limits participation to no more than 300 eligible participants, with implementation required in at least one maternity care desert county and at least one rural county as defined by the Center for Rural Pennsylvania

Provides FDA-authorized cellular-connected devices (blood pressure monitors, blood glucose meters, scales) delivered directly to participants, with monitoring available during pregnancy and up to three months postpartum

Allocates up to $600,000 in aggregate fees to participating managed care organizations to contract with a selected technology vendor and administer the program, which must be operational within 180 days of contract execution

Requires the Department of Health to submit an evaluation report within 24 months after program expiration to the Governor and legislative leaders, assessing cost savings, morbidity impacts, and recommendations for statewide expansion
