Requires health insurers in Pennsylvania to cover immunizations with no cost-sharing (no deductibles, copays, or coinsurance) for enrollees, excluding grandfathered health plans

Covers all vaccines required by individual health insurance policies as of January 1, 2025, plus any immunizations recommended by the CDC's Advisory Committee on Immunization Practices

Authorizes the Insurance Department, in consultation with the Department of Health, to add or remove immunizations from coverage requirements based on medical evidence and public health considerations

Permits insurers to impose cost-sharing for out-of-network immunizations unless no in-network provider is available, and protects enrollees claiming religious freedom exemptions

Establishes enforcement penalties including fines up to $5,000 per violation ($10,000 for willful violations), with a $500,000 annual cap, plus potential license suspension or revocation
