Requires healthcare practitioners to obtain written informed consent signed by the recipient (or parent/guardian for minors) before administering any vaccine, with consent language approved by the Director of Health that includes the right to decline and information about the National Vaccine Injury Compensation Program

Prohibits practitioners and healthcare facilities from discriminating against, dismissing, harassing, coercing, or threatening patients who delay or decline vaccinations

Prohibits insurers from denying coverage, increasing premiums, or discriminating against individuals or providers based on vaccination decisions, and prohibits employers from denying or terminating employment based on an individual's choice to delay or decline vaccination

Bans practitioners from accepting bonuses or incentives from insurance or pharmaceutical companies for vaccinating patients, and from requiring liability waivers as a condition of care when patients decline vaccines

Establishes fines up to $25,000 per patient per violation for intentional and knowing violations, allows civil recovery for actual damages, and prohibits the Department of Children, Youth and Families from investigating parents solely for choosing to delay or decline vaccinations for their children
