Requires all embryo storage facilities in Rhode Island to obtain a license from the Department of Health, with separate licenses needed for each location and annual renewal required.

Directs the Department of Health to promulgate regulations based on ISO standards, College of American Pathologists biorepository standards, and FDA tissue practice guidance covering facility operations, staff qualifications, equipment safety, and record confidentiality.

Grants the Department authority to conduct unannounced on-site inspections during normal operating hours and to request fire, health, and building code inspections within 60 days.

Authorizes license denial, suspension, or revocation for non-compliance, fraud, or refusal to permit inspections, with operators entitled to a hearing within 60 days of notice.

Establishes felony penalties for operating without a license or violating the act, punishable by fines up to $5,000, imprisonment up to 2 years, or both; effective January 1, 2027.
