Establishes a 19-member Food-as-Medicine Task Force within the Executive Office of Health and Human Services (EOHHS) to develop recommendations for a pilot program providing medically tailored meals and nutrition supports to individuals with diet-related chronic diseases.

Task force must convene within 90 days of the act's passage and submit findings and recommendations to the governor and legislative leaders by December 31, 2026.

Authorizes EOHHS to seek federal approval, including a Section 1115 Medicaid waiver, to implement the pilot program for eligible Medicaid beneficiaries with chronic, diet-related conditions.

Target populations include Medicaid enrollees, older adults, and individuals with complex medical needs who experience food insecurity and disproportionate burden of conditions like diabetes, cardiovascular disease, and obesity.

No state expenditures required beyond those authorized through federal approval, existing appropriations, or future legislative action.
