Requires Advanced Practice Registered Nurses (APRNs), Physician Assistants (PAs), and Anesthesiologist's Assistants (AAs) to practice as part of a patient care team led by a physician, regardless of practice setting or ownership structure

Mandates practice agreements and scope of practice guidelines include specific provisions for physician chart review, availability for collaboration/consultation, site visit frequency, and joint evaluation of clinical quality and outcomes

Creates a 13-member Team Based Health Care Committee (5 physicians, 2 APRNs, 2 PAs, 1 AA, 2 ex officio board members, 1 public member) to oversee team-based health care, conduct random audits of practice relationships, and recommend regulations to the Boards of Medical Examiners and Nursing

Requires disclosure to patients at initial encounter of provider credentials, the patient care team physician's name and contact information, and prohibits doctoral-level APRNs/PAs/AAs from using "doctor" title without clarifying they are not a physician

Directs the Department of Health and Human Services to issue a report within six months recommending incentives for team-based care in rural and underserved areas, including loan repayment programs, tax policies, telehealth expansion, and Graduate Medical Education funding increases
