Health insurers and TennCare plans must cover biomarker testing for diagnosis, treatment, management, or monitoring of diseases when supported by medical evidence, effective January 1, 2026

Covered biomarker tests include single-analyte tests, multi-plex panels, protein expression analysis, and whole exome/genome/transcriptome sequencing of tissue, blood, or other biospecimens

Coverage required when testing is supported by FDA-approved test indications, FDA-approved drug labels, CMS coverage determinations, or nationally recognized clinical practice guidelines

Prior authorization decisions must be made within 72 hours for non-urgent requests and 24 hours for urgent requests

Insurers must provide a clear, publicly accessible process for patients and providers to appeal coverage denials or request exceptions to coverage policies
