TennCare (Tennessee's Medicaid program) is prohibited from providing coverage or reimbursement for medical procedures intended to help individuals identify with or live as a gender inconsistent with their sex assigned at birth, or to treat distress from gender-sex discordance.

Covered procedures include surgeries modifying tissues or organs, as well as prescribing puberty blockers or hormones (androgens or estrogens) for these purposes.

Exceptions allow coverage for treating congenital defects, precocious puberty, disease, or physical injury—but gender dysphoria, gender identity disorder, and gender incongruence are explicitly excluded from the definition of "disease."

Individuals already receiving such treatment before the effective date may continue receiving TennCare coverage until March 31, 2027, provided their physician certifies in writing that stopping treatment would be harmful.
