Health insurers and TennCare plans must cover biomarker testing for diagnosis, treatment, management, or monitoring of diseases when supported by medical evidence, effective January 1, 2026

Covered biomarker testing includes analysis of tissue, blood, or biospecimens for gene mutations, protein expression, and whole genome/exome sequencing when indicated by FDA-approved tests, drugs, or nationally recognized clinical guidelines

Prior authorization decisions must be made within 72 hours for non-urgent requests and 24 hours for urgent requests

Insurers must provide coverage in a manner that limits care disruptions, including avoiding the need for multiple biopsies or biospecimen samples

Patients and providers must have access to a clear, publicly accessible process on insurer websites to request exceptions to coverage policies or appeal adverse decisions
