Out-of-network ambulance service providers are prohibited from balance billing health plan enrollees for covered emergency ambulance services, effective for plans issued or renewed on or after July 1, 2025

Enrollee cost-sharing is capped at the lesser of: the in-network cost-sharing amount, the provider's billed charges, or 325% of the Medicare rate for the same service in the same geographic area

Health insurers must apply enrollee payments for out-of-network ambulance services toward in-network deductibles and out-of-pocket maximums

Ambulance service organizations are prohibited from requiring enrollees to sign waivers that would void or alter these consumer protections
