Designates December 14 as "Hazel Moore Day" in Tennessee to honor the Whitehaven community leader known as the "Mayor of Whitehaven"

Moore founded the Academy of Youth Empowerment to help teens develop social skills and study habits, and led efforts to build a new library

Moore established the Community Health Fair providing free school supplies and health screenings for students and parents

Awards received include the Pioneer Memphis Business Award (1996), American Heart Association Outstanding Volunteer Award (1998, 2001), and Black Business Association's Benny Award (2002)

A street and an award have been named in her honor for contributions to the Memphis community
